29 CFR 1910.305(g)(1)(iv)(D):Flexible cords were attached to building surfaces:  On or about December 18th, 2025, employees engaged in production activities located at 915 North 43rd Ave, Omaha, Nebraska 68131, were exposed to fire hazards in that a flexible cord was supplying power to a fan that was facing Line 4. The flexible cord had been plugged in continuously for at least year and it was attached to building surfaces.
