29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about December 12, 2025, the adjustable guard of the AEW 400 series, 3" bandsaw located in the pork area at the facility was not fully extended to cover the entirety of the blade. Push sticks and/or other devices to protect against inadvertent contact with the unguarded blade were not provided and/or their use enforced by the employer potentially exposing employees feeding raw material/stock into the point of operations potentially to amputation hazards.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about December 12, 2025, the employer failed to report a work related hospitalization within 24 hours of the injury occurring. The event occurred on December 12, 2025 at approximately 2:45 PM and OSHA was not notified until December 15, 2025 at 6:30 PM.
