Material removed from the roof was dropped or thrown to the ground without the use of an enclosed chute, crane, or hoist, potentially striking employees, or individuals below. The drop distance was approximately 30 feet.  Occurred/witnessed on January 02, 2026

Employees were observed leaning over the roof edge without utilizing any form of fall protection, including guardrail systems, safety net systems, personal fall arrest systems, or a combination of a warning line system and a safety monitoring system, as required by the standard.
