29 CFR  1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s).  A) Myles Construction, LLC. - 508 1st Ave., Elizabeth NJ 07206 - On or about 02 Jan 2026, at the work site located at 1030 St. Georges Ave., Avenel NJ 07001, employees engaged in roofing activities were exposing others to struck-by hazards from falling debris. Material removed from the roof was dropped or thrown to the ground without the use of an enclosed chute, crane, or hoist, potentially striking employees, or individuals below. The drop distance was approximately 30 feet.  Occurred/witnessed on January 02, 2026

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net(s).  A) Myles Construction LLC. - 1030 St. Georges Ave., Avenel, NJ 07001 -  Employees engaged in roofing activities on a low-slope roof with unprotected sides and edges approximately 30 feet above a lower level were exposed to fall hazards. Employees were observed leaning over the roof edge without utilizing any form of fall protection, including guardrail systems, safety net systems, personal fall arrest systems, or a combination of a warning line system and a safety monitoring system, as required by the standard.  Condition observed on December 31, 2025
