29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) On December 23, 2025 at Marinenko 145-97-30B-31-3H wellsite near Killdeer, North Dakota, employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries from a release of hazardous stored energy such as but not limited to,  mechanical and hydraulic energy.  Employees perform inspection and maintenance activities such as, but not limited to, applying adhesive spray to belts and checking belt tensions on a pumping unit without having or utilizing procedures to control all hazardous energy sources.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a) On December 23, 2025 at Marinenko 145-97-30B-31-3H wellsite near Killdeer, North Dakota, employees were exposed to struck-by and caught-in-between hazards that could result in serious injuries from the release of hazardous stored energy such as, but not limited to hydraulic and mechanical energy.  The employer did not follow methods of control for [1910.147(d)(1)]; orderly shutdown [1910.147(d)(2)]; locating/operating energy isolating devices [1910.147(d)(3)]; lock-out/tag-out device application [1910.147(d)(4)]; dissipation of residual energy [1910.147(d)(5)]; and verification of isolation [1910.147(d)(6)] were not implemented prior to inspection, maintenance, and service activities.

(a) On December 23, 2025, at Marinenko 145-97-30B-31-3H wellsite near Killdeer, North Dakota, employees were exposed to struck-by, caught-in-between hazards that could result in serious injuries when conducting inspection and maintenance activities on a pumping unit such as, but not limited to, applying adhesive spray to belts and checking belt tensions, the employer did not train employees on the methods and means necessary for energy isolation, hazardous energy recognition, and control of the Luftkin Pumping unit.
