1926.21(b)(2)  The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. 29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  LOCATION: The jobsite located at 15 4th St. Chelsea, MA  On 12/31/2025 and days prior the employer did not ensure that each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.

29 CFR  1926.761(c)(3): Where controlled decking zones (CDZ) are being used, the employer did not assure that each employee had been provided training in the nature of the hazards associated with work within a CDZ and the establishment, access, proper installation techniques and work practices required by §1926.760(c) and §1926.754(e).   LOCATION: The jobsite located at 15 4th St. Chelsea, MA  On 12/31/2025 and days prior the employer did not ensure that their workers were properly trained in the establishment, hazards, or installation of a controlled decking zone.
