1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: a) Roof Area - On or about December 19, 2025, the employer exposed employees to amputation hazards, in that the employer failed to ensure that special hand tools for placing and removing materials were provided to employees for easy handling of materials without placing their hands in the danger zone while employees involved in activities such as threading and tensioning belts during positioning stage of belt replacement on a Cincinnati Fan (Model #RBE-21 - Serial #1208523).
