29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting concrete with a saw, without the use and wear of protective eyewear:  a.  At a temporary worksite, located at the east side of  11938 Generation  Ave Lot 286 On or about January 7, 2026, the employer exposed employees to eye-injury struck-by hazards, in that protective eyewear was not utilized by employees while using metal hammers and nail guns for siding work.

29 CFR  1926.451(b)(6):  On scaffolds where scaffold planks are abutted to create a long platform, each abutted end shall rest on a separate support surface. This provision does not preclude the use of common support members, such as "T" sections, to support abutting planks, or hook on platforms designed to rest on common supports.   a.  At a temporary worksite, located at the east side of  11938 Generation  Ave Lot 286 On or about January 7, 2026, the employer was exposing  employees to a 10-11 foot fall hazard, in that the employees worked from a scaffold plank that was abutted to another plank and one end did not rest on a ladder jack support of the ladder jack scaffold system.

At a temporary worksite, located at the east side of  11938 Generation  Ave Lot 286 On or about January 7, 2026, the employer was exposing  employees to a 10-11 foot fall hazard, in that the employees worked from bridged scaffold planks on a ladder jack scaffold.

At a temporary worksite, located at the east side of  11938 Generation  Ave Lot 286 On or about January 7, 2026, the employer was exposing  employees to a 10-11 foot fall hazard, in that the employees were not tied off when conducting siding work from a ladder jack scaffold and there were no other means of fall protection from the platform.
