29 CFR 1926.1402(b): The equipment was assembled or used without ensuring that ground conditions are firm, drained, and graded to a sufficient extent so that, in conjunction (if necessary) with the use of supporting materials, the equipment manufacturer's specifications for adequate support and degree of level of the equipment are met:  a) Worksite, mobile crane: On or about July 22, 2025, employees were assembling a Liebherr LTM 1220 mobile crane and the employer did not ensure that the ground conditions were graded to a sufficient extent.

29 CFR 1926.1427(b)(4)(ii): While monitoring the operator-in-training, the operator's trainer performed tasks that detract from the trainer's ability to monitor the operator-in-training:  a) Worksite, mobile crane: On or about July 22, 2025, an operator-in-training was operating a Liebherr LTM 1220 mobile crane while the operator's trainer was sitting in a vehicle and was not monitoring the operator-in-training.
