29 CFR 1910.26(d): The employer must ensure that each dockboard used meets the requirements of this section. The employer must ensure measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semi-trailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard. On or about January 9, 2026, at an establishment located at 1717 Lawrence Drive in De Pere, Wisconsin; employees unloading the back of a semi-trailer by hand were exposed to trip and fall hazards from the back of the trailer when the trailer's wheels were not chocked and the sand shoes were not set to prevent the transport vehicle on which the dockboard was placed.

On or about January 5, 2026, to January 9, 2026, at an establishment located at 1717 Lawrence Drive in De Pere, Wisconsin, in the warehouse and dock areas; the employer did not ensure that the 72-inch fork extensions used on the Linde E20 electric forklifts were identified as an attachment, and the approximate weight of the truck and attachment combination at maximum elevation with the load laterally centered was marked on the truck.

On or about January 9, 2026, at an establishment located at 1717 Lawrence Drive in De Pere, Wisconsin; employees were exposed to struck-by and crushed-by hazards when a forklift operator dismounted a Linde E20 electric forklift (Serial: A1Y346L00468) to remove a piece of loose cardboard and did not fully lower the load engaging means.

On or about January 9, 2026, at an establishment located at 1717 Lawrence Drive in De Pere, Wisconsin; the employer did not ensure only safely arranged loads were handled when employees utilized a Linde E20 electric forklift (Serial: A1Y346L00468) to move pallets loaded with furniture throughout the dock loading area and warehouse. The load was visibly sagging at the center when an employee was moving a loaded pallet that was approximately 76 inches long with the forklift that was equipped with 42-inch-long forks.
