29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a) On or about January 08, 2026, at jobsite 2400 NE 44 Ct, Lighthouse Point, FL 33064, the employer did not ensure that employees performing roofing work near skylights used a personal fall arrest system, exposing employees to an approximate 8-foot fall hazard through holes, including skylights, to the ground below.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about January 08, 2026, at jobsite 2400 NE 44 Ct, Lighthouse Point, FL 33064, the employer did not ensure that its employees and the employees of its subcontractor who were working on a single-story residential roof were protected from a fall hazard of approximately 13 feet 4 inches to the ground by means of fall protection.
