On or about January 14, 2026, at the Warehouse: employees were exposed to amputation hazards, in that the INDYCNC sliding table saw model STS-45E was being operated without a blade guard.

29 CFR  1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:  a. On or about January 14, 2026, at the Warehouse: employees were exposed to electrical and fire hazards, in that, the INDYCNC sliding table saw model STS-45E was being operated while it had exposed 380V electrical wires.
