29 CFR  1910.23(c)(11): (11) Portable ladders used to gain access to an upper landing surface did not have side rails that extended at least 3 feet (0.9 m) above the upper landing surface:   a) Jobsite, 1424 Western Avenue, Pittsburgh, PA: On or about January 13, 2026 during HVAC preventive maintenance activities, the employer did not ensure the ladder used to get on/off the roof was properly set up in that it's siderails only extended approximately 2 feet above the landing surface.
