OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  USS Mustin, 3-370-0-E, #3 Generator Room: On or about January 13, 2026, and at times prior thereto, employee(s) used a Harrington 3/4 ton Lever Operated Chain Hoist which was equipped with hooks that were manufactured with safety latches, the safety latch was missing. This condition potentially exposed employee(s) to struck by and/or crush by hazard(s).

USS Mustin, 3-370-0-E, #3 Generator Room: On or about January 13, 2026, and at times prior thereto, the employer did not assess its work activity to determine whether hazards were present after employee(s) were tasked to remove and replace the #3 generator resilient mounts. This condition potentially exposed employee(s) to struck-by and/or crush-by hazard(s).
