On or about January 14, 2026, at the construction site an employee was working in a trench while installing a Hi-tap valve for a pvc water line pipe without adequate cave-in protection in place.

29 CFR  1926.1433(d)(4)(ii): The employer did not take hooks that were defective out of service.  On or about January 14, 2026, at the construction site employees were utilizing a truck mounted auto crane to install a Hi-Tap valve for pvc water pipes and the safety latch of the hoist was defective.
