29 CFR  1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures in any workplace where respirators were necessary to protect the health of the employee or whenever respirators were required by the employer.  a. On or about January 15, 2026, in the painting and welding department: employees were exposed to respiratory hazards, in that, the employer failed to establish and implement a written respiratory protection program with worksite-specific procedures for employees using Draeger - X-plore 5500 Full Face Mask respirators during various operations.

29 CFR  1910.134(f)(1): The employer did not ensure that employees using a tight-fitting facepiece respirator pass an appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT) as stated in this paragraph:   a.  On or about January 15, 2026, in the painting and welding department: employees were exposed to respiratory hazards while wearing Draeger - X-plore 5500 Full Face respirator, in that, the employees had not been fit tested to wear a respirator.
