29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about January 7, 2026, employees used an extension ladder  which extended approximately 2 feet past the landing surface, to access the roof while  performing framing activities and were exposed to falls of approximately 12 feet 7  inches to the surface below.  (b) On or about January 7, 2026, employees used a collapsed A-Frame ladder which extended approximately 1 foot 6 inches past the landing surface. Employees used it to access the second level from the first floor roof while performing framing activities and were exposed to falls of approximately 8 feet to the first floor roof below.
