29 CFR  1926.501(b)(4):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  Worksite located at Clarandon Road, Sedalia, Missouri- Employees were exposed to fall hazards while working next to hole openings in the floor and were not protected from falling by the use of any means of fall protection. On or about January 9, 2026 an employee fell from a unprotected floor hole on the 2nd level of the mezzanine.

Covers for all the openings were not secured from accidental displacement.
