29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) On or about January 16, 2026, at jobsite location 4839 NW 112nd Drive Coral Springs FL 33076, employees engaged in roofing work at a residential roof with a slope of 6:12 were exposed to a fall hazard of approximately 10-feet above ground level without a means of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) On or about January 16, 2026, at jobsite location 4839 NW 112nd Drive Coral Springs FL 33076, employees were using a 12 feet aluminum multiuse ladder that was not secured at its top to a rigid support, upper level or landing and not extended 3 feet exposing employees to a fall hazard.
