29 CFR  1926.960(c)(1)(i): The employer did not establish minimum approach distances (MAD) no less than the distances computed by Table V-2 for ac systems:  a) 743 Wyoming Street Butte, MT 59701: On or about January 12th, 2026, and at times prior, employees were exposed to electric shock hazards while setting trusses in direct proximity to a 7,200 volt live electrical distribution line. The employer failed to establish minimum approach distance prior to beginning the work that would bring the crane in proximity to the power line.
