At a jobsite, located at 7410 Merrill Road, Jacksonville, FL: On or about January 24, 2026, the employer exposed employees to a struck-by hazard, in that an employee discharging tree limbs and debris into a Bandit Intimidator woodchipper was not wearing safety glasses .

At a jobsite, located at 7410 Merrill Road, Jacksonville, FL: On or about January 24, 2026, the employer exposed employees to overhead hazards, in that ground personnel working underneath tree trimming operations retrieving cut tree limbs and debris were not wearing protective helmets.  b.  At a jobsite, located at 7410 Merrill Road, Jacksonville, FL: On or about January 24, 2026, the employer exposed employees to overhead hazards, in that two employee conducting tree trimming operations was not wearing a protective helmet.
