29 CFR 1910.147(c)(4)(ii):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including but not limited to, those regulations found within 29 CFR 1910(c)(4)(ii)(A)-(D):   a) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not clearly and specifically include the authorization to execute the procedure, and the means to enforce compliance with the procedure, while employees were conducting machine set-up and/or repairs, on or about January 29, 2026.  b) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not include a scope, purpose and/or specific statement of the intended use of the procedure while employees were conducting machine set-up and/or repairs, on or about January 29, 2026.  c) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not include specific procedural steps and techniques for the removal of lockout devices and/or tagout devices, and the responsibility for them, while employees were conducting machine set-up and/or repairs, on or about January 29, 2026.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their  representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i):  The employer did conduct a periodic inspection of the energy control procedure at least annually but did not ensure that the procedure and the requirement of this standard were being followed:  a) 505 East Fulton Street Lancaster, PA 17602 - Employees were conducting service and maintenance on machinery, including, but not limited to, the Alliance #49 (ID# 000587), and the periodic inspections of the energy control procedures were group together as one audit containing machinery with various methods of energy control and energy sources, on or about January 29, 2026.

29 CFR 1910.212(a)(3)(ii):  Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) 505 East Fulton Street, Lancaster, PA 17602 - Employees were exposed to point of operation hazards while operating the acid brush trimmer, as the employees' unprotected hands were within 6-inches of the unguarded cutting blade while trimming 6-inches acid brushes, on or about January 29, 2026.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1):  The employer did not enter information about the business at the top of the OSHA 300 Log, a one or two line description for each recordable injury or illness, and a summary of this information on the OSHA 300-A at the end of the year.  a)  505 East Fulton Street Lancaster, PA 17602 - on or about January 29, 2026, Column F for Case dated 10/22 on the 2025 OSHA Form 300 Injury and Illness Log provided the injury and part of the body affected, but did not include the object/substance that directly injured the person.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
