29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including but not limited to, those regulations found within 29 CFR 1910(c)(4)(ii)(A)-(D): a) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not clearly and specifically include the authorization to execute the procedure, and the means to enforce compliance with the procedure, while employees were conducting machine set-up and/or repairs, on or about January 29, 2026. b) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not include a scope, purpose and/or specific statement of the intended use of the procedure while employees were conducting machine set-up and/or repairs, on or about January 29, 2026. c) 505 East Fulton Street Lancaster, PA 17602 - The energy control procedures for machinery including, but not limited to, the Alliance #28 (Machine ID# 000588) and the Alliance #49 (ID# 000587), did not include specific procedural steps and techniques for the removal of lockout devices and/or tagout devices, and the responsibility for them, while employees were conducting machine set-up and/or repairs, on or about January 29, 2026. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
