29 CFR  1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use:   (a) The door to to the deck outside of the break room, on or about January 27, 2026, exposed employees to injury because it was not clearly marked "Not an Exit" and the door leading into this room (219) also was not marked as "Not an Exit".

29 CFR  1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  (a) In the main hallway, on or about January 27, 2026, and at times prior thereto, employees were exposed to safety hazards  because the portable fire extinguisher was not being inspected on a monthly basis.
