29 CFR  1910.37(b)(4):Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  (a) North end of the office: On or about December 8, 2025 and at times prior thereto, office employees were exposed to smoke inhalation and minor burn hazards because the direction of travel to the exit was not immediately apparent and there were no signs indicating  the direction of travel to the nearest exit.

29 CFR  1910.37(b)(5):Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  (a) Northeast corner of building: on or about December 8, 2025 and at times prior thereto, office employees were exposed to smoke inhalation and minor burn hazards because every door which could be mistaken for an exit door was not clearly marked as  "Not an Exit"  or identified by a sign indicating its actual use, including an unmarked non-exit door immediately opposite to an unmarked exit door.

29 CFR  1904.40(a):The employer did not provide an authorized government representative the requested records within four hours:  (a) at the Longview, WA  SSA building, on or about December 8, 2025, the employer did not provide OSHA 300 logs and 300A annual summaries for 2023 to an authorized government representative within four business hours, which is a regulation implemented to reduce employee exposure to hazards.
