29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed:  a) On or about January 29, 2026, at jobsite location 6950 Northwest 77th Court Miami FL 33166, the employer did not ensure employees operating powered industrial truck sounded the horn when entering the freezer area at the warehouse, thereby exposing employees to a struck-by hazard.

29 CFR  1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:  a) On or about January 29, 2026, at jobsite location 6950 Northwest 77th Court Miami FL 33166, the clear plastic strips curtain became foggy due to condensation thus reducing visibility of the forklift operators and exposing employees to a struck-by hazard.
