OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hand entanglement and entrapment hazards while wearing gloves when operating a Grizzly Industrial, Inc. Model G0495X 8" x 83" Jointer with Digital Height Readout:  On or about January 28, 2026, the employer did not protect employees that operate the Grizzly Industrial, Inc. Model G0495X 8" x 83" Jointer with Digital Height Readout, Serial #B06/328, from hand entanglement and entrapment hazards by ensuring that they remove any wearable items such as gloves. An employee wore gloves while operating the jointer and sustained a three-finger amputation when a glove contacted the cutterhead.

29 CFR 1904.39(a)(2): The employer did not report within 24 hours an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident:  On or about January 28, 2026, the employer did not report a work-related amputation to OSHA within 24 hours. The incident happened at approximately 2:15 p.m. MDT on January 28, 2026, and was not reported to OSHA until January 29, 2026, at 4:02 p.m. MDT.
