Location: 309 East Fordham Road, Bronx, NY  a) On or about February 12, 2026, the employer did not record the workplace illness on the OSHA Form 300, or its equivalent, for the calendar year 2026. An employee suffered an illness from exposure to cleaning chemicals, including but not limited to bleach, Lynx RTU Quat cleaner disinfectant and RJ8, resulting in days away from work. The employer was made aware of all information necessary to record this injury on or about January 26, 2026.
