29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  (a) Worksite - JLG Lift: On or about 2/5/2026, an employee was exposed to fall hazards while working from a elevated JLG lift without fall protection. The employee was exposed fall hazards of approximately 25 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) Worksite - Building 3: On or about 2/5/2026, employees were exposed to fall hazards while performing residential construction without fall protection. Employees were exposed to falls of approximately 10 feet due to unguarded floor openings. (b) Worksite - Building 3: On or about 2/5/2026, employees were exposed to fall hazards while performing residential construction without fall protection. Employees were exposed to falls of approximately 20 feet due to unguarded wall openings.

(a) Worksite - On or about 2/5/2026, the employer could not provide a written certification of training for employee fall protection.
