29 CFR 1910.140(c)(18): Personal fall protection systems must be inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components must be removed from service.  a. On or about February 5, 2026, the employer did not ensure that employees were inspecting their fall protection harnesses. As a result, an employee was operating an order picker forklift with a Miller Titan T4000/UAK, WO 2023891, fall protection harness that had worn and deteriorated straps near the buckles.
