29 CFR  1904.40(a): The employer did not provide an authorized government representative requested injury and illness records within four business hours.  Thomas Creek Woodworks failed to provide their OSHA 300 Logs and Form 300A for their facility located at 7050 North 97th Circle Plaza OMAHA, NE 68122 to an authorized OSHA representative within four hours of the representative making both an oral informal and a formal written request for said documents. Instances of failure to respond include but are not limited to:  i. On or about February 5, 2026, when OSHA made a verbal request to the company during the opening conference and site walkaround.  ii. On or about February 5, 2026, when OSHA made a written request to the company via email, quoting this requirement from the 1904.40(a). iii. On or about February 6, 2026, when OSHA made another written request to the company via email.

29 CFR  1910.1200(e)(1): Employer had not implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  On or about February 5, 2026, employees engaged in commercial woodworking activities, located at 7050 North 97th Circle Plaza, OMAHA, NE 68122, were exposed to chemical hazards in that the employer had not developed a written hazardous communication program.
