29 CFR  1910.106(d)(3)(ii)(a): Metal cabinets constructed in the following manner shall be deemed to be in compliance. The bottom, top, door, and sides of cabinet shall be at least No. 18 gage sheet iron and double walled with 1 1/2 - inch air space. Joints shall be riveted, welded or made tight by some equally effective means. The door shall be provided with a three-point lock, and the door sill shall be raised at least 2 inches above the bottom of the cabinet.  Acton Fire Station, south end outside the building: On or about February 10, 2026, and at times prior thereto, the yellow metal cabinets which contained flammable chemicals was damaged and did not close adequately with a proper seal. This condition potentially exposed employee(s) to fire hazard(s).

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  Acton Fire Station: On or about February 10, 2026, and at times prior thereto, all the fire extinguisher(s) mounted/staged throughout the building were not up to date with their annual maintenance check. This condition potentially exposed employee(s) to fire hazard(s).  Notice 1, Item 1, 29 CFR 1910.157(e)(3) as required by 29 CFR 1960.8(b): Portable fire extinguishers were not subjected to an annual maintenance check. The employer was cited for a violation of the same standard on July 28th, 2025, Inspection 1827381, August 21, 2025, of final order/settlement, and means of abatement.

29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5):  Acton Fire Station, break room: On or about February 10, 2026, and at times prior thereto, a plastic spray bottle which contained Murphy's wood cleaner was not labeled with a Globally Harmonized System Label. This condition potentially exposed employee(s) to chemical hazard(s).
