29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that met the recording criteria on the OSHA Form 300 or equivalent: a) 3885 Columbia Avenue, Mountville, PA 17554, Recordkeeping - On or about December 3, 2025, an employee was injured when they fell while exiting the truck resulting in 2 days away from work and the employer did not record the workplace injury on the OSHA Form 300 or equivalent for calendar year 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
