29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. 29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.    On or about February 10, 2026, the employer did not certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards of approximately 14 feet while performing roofing activities without using fall protection.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:  The employer does not protect each employee required to work from elevations during residential construction activities 6 feet or more above lower levels by ensuring the use guardrail systems, safety net system, personal fall arrest system, or providing alternative fall protection measures under provisions of paragraph 1926.501(b):  This most recently occurred On or about February 10, 2026, employees were exposed to fall hazards of approximately 14 feet while engaged in residential construction activities. Fall protection was not being utilized while employees were performing roofing work.   A&J Roofing Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1707008, citation number 2, item number 1 and was affirmed as a final order on March 13, 2024, with respect to a workplace located at 340 Green St., Somonauk, IL. 60552.  A&J Roofing Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1679840, citation number 2, item number 2 and was affirmed as a final order on October 31, 2023, with respect to a workplace located at 3024 Secretariat Ln., Montgomery, IL. 60538  A&J Roofing Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1619403, citation number 2, item number 1 and was affirmed as a final order on December 8, 2022, with respect to a workplace located at 2256-1-2256-13 Oak Ridge Dr., Aurora, Il. 60504.
