29 CFR  1904.39(a)(2): The employer did not report within 24-hours an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident.  (a)  On or about January 7, 2026 an employee was injured at a jobsite located at W124N8925 Boundary Road, Menomonee Falls, WI (WM Orchard Ridge), which resulted in a finger amputation.  The employer did not report the amputation to OSHA within 24 hours of learning of the amputation.
