On January 2, 2026, employees working in the administrative office, quality control laboratory, and the oil production room were exposed to excess levels of carbon monoxide from multiple sources, including but not limited to propane heaters, a propane powered forklift, and a propane liquid transfer pump.  1. An employee was exposed to 116.8 parts per million (ppm) carbon monoxide level of air as determined by the employee's Carboxyhemoglobin (%COHb), which was 15.59% medical blood gas measurement following the employee's exposures to carbon monoxide. The 8-hour Time Weighted Average (TWA) exposure was 110.7 ppm CO (Sampling and Analytical Error SAE = 0.162), which is 2.21 times the OSHA PEL of 50 ppm CO.  2. A employee was exposed to 238.0 parts per million (ppm) carbon monoxide level of air as determined by the employee's Carboxyhemoglobin (%COHb), which was 27.26% medical blood gas measurement following the employee's exposures to carbon monoxide. The 8-hour TWA exposure was 238.0 ppm CO (Sampling and Analytical Error SAE = 0.153), which is 4.76 times the OSHA PEL of 50 ppm CO.  3. A employee was exposed to 458.6 parts per million (ppm) carbon monoxide level of air as determined by the employee's Carboxyhemoglobin (%COHb), which was 36.34% medical blood gas measurement following the employee's exposures to carbon monoxide. The 8-hour TWA exposure was 257.9 ppm CO (Sampling and Analytical Error SAE = 0.092), which is 5.16 times the OSHA PEL of 50 ppm CO.
