29 CFR  1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel, and provide contractors access to these safe work practices:  (a) On and or about February 4, 2026, the contract employer did not ensure that their employees were provided access to the host employer's safe work practices and procedures for the ammonia refrigeration system at 9501 Nevada Ave., Franklin Park, IL 60131, while the contractor's employees performed a shaft seal change on compressor SC-3, which required the contractor to perform a line break procedure, and required the host employer's employee to accompany their employee in accordance with the buddy system.  (b) On and before February 24, 2026, the employer did not develop and implement safe work practices to ensure the control of fall hazards for employees during operations on the ammonia refrigeration system at 9501 Nevada Ave, Franklin Park, IL 60131, which required line breaks on equipment which could only be accessed by extension ladders in the facility and required the use of 2 hands to perform such work.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
