29 CFR  1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds).  Construction Reference 29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 18, 2026, at 12900 Snow RD, Parma Ohio 44130, and at times before, the employer did not ensure that the safety chains (end chains to complete the guardrail system of the man basket of a scissor lift) were properly secured to prevent an employee from falling 12 to 15 feet to the ground from the elevated work platform.

29 CFR  1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  On or about February 23, 2026, at 12900 Snow RD, Parma Ohio 44130, during the loading/unloading and material handling process employees utilized Power Industrial Trucks/Vehicles (Scissor Lifts, Fork Trucks and Tow Motors) without being properly trained.

29 CFR  1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:  On or about February 23, 2026, at 12900 Snow RD, Parma Ohio 44130, the employer failed to ensure the chains utilized to support the loading/unloading graphite powder (up to 2,000 lbs bags) had a durable affixed identification tag. The exposed employees utilized the alloy steel chain continuously to load/unload the graphite super sacs during the sifting and re-bagging process..
