29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds). Construction Reference 29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about February 18, 2026, at 12900 Snow RD, Parma Ohio 44130, and at times before, the employer did not ensure that the safety chains (end chains to complete the guardrail system of the man basket of a scissor lift) were properly secured to prevent an employee from falling 12 to 15 feet to the ground from the elevated work platform.

The exposed employees utilized the alloy steel chain continuously to load/unload the graphite super sacs during the sifting and re-bagging process..
