29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i).  PMC Northern Victor, Deck B:  On or about  February 26, 2026, and at times prior thereto, employee(s) were tasked to dump fish into the auto?grinder from a height of 6.5 feet were not provided with adequate side or edge protection. This condition exposed employee(s) to a fall hazard .

PMC Northern Victor, Switch room: On or about  February 26, 2026, and at times prior thereto, employee(s) were tasked to maintain circuit breaker panels. A (460/480) - volt circuit breaker panel had an unused opening, exposing the energized components inside the circuit breaker panel. This  exposed employee(s) to electrical hazard(s).
