29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  Building 1600, Weld Shop: On or about March 18, 2026, and at times prior thereto, frequent inspections were not conducted for a Sheppard Niles 5-ton overhead crane. The crane was observed with a cable that had derailed from its track and contained multiple kinks.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Building 1600, 3D printer room: On or about March 9, 2026, and at times prior thereto, electronic equipment was plugged into a Tripp-Lite relocatable power tap, which was plugged into a black extension cord and not used according to listing/ labeling instructions.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  Building 1600, 3D printer room: On or about March 9, 2026, and at times prior thereto, the industrial rack systems were used to store 3D printing supplies and were not anchored with anchor bolts to the concrete floor.
