29 CFR  1926.552(a)(1): The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:  a) On or about March 10, 2026, at the worksite, 203 Onondaga Ave, Syracuse, NY 13207: The employer required employees to use the tracks of a material hoist as a ladder, disregarding manufacturer limitations.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about March 10, 2026, at the worksite, 203 Onondaga Ave, Syracuse, NY 13207: The employer failed to ensure employees replacing a roof, exposed to fall hazards, were protected from falling by use of personal fall arrest systems, safety net systems, or guardrail systems.  Lozano General Contractor Inc DBA Mike Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1829419, citation number 2, item number 1 and was affirmed as a final order on September 23, 2025, with respect to a workplace located at 314 Smelkoff Rd., Syracuse NY 13209.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about March 10, 2026, at the worksite, 203 Onondaga Ave, Syracuse, NY 13207: The employer failed to ensure the side rails of the portable ladder employees were using to access the roof were extended three feet above the access point.  Lozano General Contractor Inc DBA Mike Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.1053(b)(1), which was contained in OSHA inspection number 1829419, citation number 1, item number 1 and was affirmed as a final order on September 23, 2025, with respect to a workplace located at 314 Smelkoff Rd., Syracuse NY 13209.
