29 CFR  1910.22(a)(3): Walking-working surfaces are not maintained free of hazards such as loose boards and unstable walking surfaces.  a) On or about March 17, 2026, the employer did not ensure that the walking-working surface at the wet end on the corrugator bridge was maintained free of hazards. Specifically, sections of the bridge belt area consisted of wooden boards that were worn, deteriorated, and in a degraded condition, creating unstable walking-working surfaces. As a result, employees performing operational and maintenance tasks in this area were exposed to unstable walking-working surfaces, creating a trip and loss of footing hazard.  b) On or about March 17, 2026, the employer did not ensure that the walking-working surface on the mezzanine was maintained free of hazards. Specifically, sections of the flooring were not properly supported and contained gaps, and a board at the top of the stairway was loose and unstable under foot, resulting in unstable walking-working surfaces. As a result, employees performing material handling tasks in this area were exposed to unstable walking-working surfaces, creating a trip and loss of footing hazard.

29 CFR  1910.29(b)(3): (3) Guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail;  a)	On or about March 17, 2026, the employer did not ensure that guardrail posts at the wet end on the corrugator bridge were properly secured to the walking-working surface. Specifically, the guardrail base plates were missing anchoring bolts, with visible empty bolt holes, resulting in unstable guardrails that were not capable of withstanding the required 200-pound force. As a result, employees performing operational and maintenance tasks in this area were exposed to fall hazards of approximately 10 feet to the lower level.  b)	On or about March 17, 2026, the employer did not ensure that guardrail posts at the rear side of the mezzanine were properly secured to the walking-working surface. Specifically, the guardrail base plates were missing anchoring bolts, with visible empty bolt holes, resulting in unstable guardrails that were not capable of withstanding the required 200-pound force. As a result, employees accessing this area to store and retrieve materials were exposed to fall hazards of approximately 10 feet to the lower level.
