29 CFR 1910.67(c)(2)(v): A personal fall arrest of travel restraint system that meet the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  On or about 03/25/2026, at1388 NW Commerce Center Drive, Port Saint Lucie, Florida, employee engaged in cable installation work was exposed to an approximated 18-foot fall hazard while working from an aerial lift without the required fall protection equipment such as a full body harness or belt with a lanyard attached to the boom or basket.
