1910.333(a): "General." Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:  a. On or about March 26, 2026, two employees performed work to replace arresters under energized conditions at the 2.1A transformer on Loop 3 at the worksite. The employer's developed and trained safe work practices and procedures were not utilized onsite. As a result, employees were exposed to electrical shock hazards while working near energized electrical parts that had not been deenergized and while performing work without utilizing the required electrical protective equipment that was available.

29 CFR  1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.  Note: Personal protective equipment requirements are contained in subpart I of this part.  a.  On or about March 26, 2026, the employer failed to ensure that employees were protected from electrical shock hazards when they when they performed work on or near energized parts to replace the arresters for the 480/277 volt, 3 phase, 4 wire, 2800 kVA transformer.  Employees worked in the proximity to the energized equipment without the required electrical protective equipment.  An employee sustained electrical shock injuries as a result.
