Remote warehouse: On or about April 23rd, 2026, and at times prior thereto, employee(s) used a Yale electric forklift to load/unload product(s). The employer did not ensure removal of the power industrial truck(s) that were not in safe operating condition(s).  Following equipment/non-functional devices:  a) Yale Forklift Truck, Model No. ERC040VAN36TE082 (Both the right and left brake lights are inoperable)

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about April 23rd, 2026, and at times prior thereto, the employer did not ensure that electrical equipment was used or installed in accordance with instructions included in the listing or labeling. Multiple appliances such as Refrigerator and a Microwave oven  were plugged into a relocatable power tap in which were not used according to listing and labeling instructions.  In the following location(s): a) Administrative office - Refrigerator b) Employee breakroom - Microwave oven and Coffee maker  Notice 1, Item 9, 29 CFR 1910.303(b)(2) as required by 29 CFR 1960.8(b): The employer did not ensure that electrical equipment was used or installed in accordance with instructions included in the listing or labeling. The employer was cited for a violation of the same standard on October 8, 2024, Inspection 1780247, November 29, 2024, of final order/settlement, and means of abatement.

29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  Meat department: On or about April 23rd, 2026, and at times prior thereto, the employer did not ensure at least 3 feet in front of the electrical panel was clear. The circuit breaker panel had a green epoxy drying rack obstructing the panel. This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency.  Notice 1, Item 10, 29 CFR 1910.303(b)(2) as required by 29 CFR 1960.8(b): The employer did not ensure that electrical equipment was used or installed in accordance with instructions included in the listing or labeling. The employer was cited for a violation of the same standard on October 8, 2024, Inspection 1780247, November 29, 2024, of final order/settlement, and means of abatement.

The employer's Emergency Action Plan did not specify the path or route of travel for emergency evacuation.
