29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a) At 3658 Lawrenceville Highway, Tucker, GA 30084, Excavation: On or about 5/5/2026, the employer exposed employees to engulfment/cave-in hazards while working inside an excavation that measured 6-feet deep, in that the spoil pile was not placed 2-feet away from the edge while engaged in manhole and sewer installation activities.
