29 CFR  1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.  Location: 166 Magazine Street Cambridge MA  On or about 5/8/2026, employees voluntarily donning tight fitting half face N95 were not medically cleared prior to donning the respirator.

29 CFR  1910.1001(d)(2)(i): The employer did not perform initial monitoring of employees who were, or might reasonably be expected to be exposed to airborne concentrations at or above the 8-hour TWA and/or excursion limits:  Location 166 Magazine Street Cambridge MA  On or about 5/8/2026, initial monitoring was not performed for work consisting of removing potentially asbestos containing siding materials to determine employee exposure.
