29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Lot 130 at 3169 Misty Springs Path, Martin Springs subdivision, Lawrenceville, GA 30045, Scaffold: On or about May 13, 2026, the employer exposed employees to fall hazards of approximately 13 feet to the lower level, in that employees were not provided fall protection while working from the pump jack scaffold system engaged in siding activities.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  a) Lot 130 at 3169 Misty Springs Path, Martin Springs subdivision, Lawrenceville, GA 30045, Scaffold: On or about May 13, 2026, employees performing siding activities from pump jack scaffolds had not been trained to recognize fall hazards while engaged in siding work at heights approximately 13 feet above the lower level.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) Lot 130 at 3169 Misty Springs Path, Martin Springs subdivision, Lawrenceville, GA 30045: On or about May 13, 2026, the employer failed to inspect all electrical extension cords used to provide power to electric tools which employees engaged in siding operations were using, in that the black cord powering the Metabo circular saw was damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
