No. 10A-1970: Barge Harvey- On or about, May 19, 2026, and at times prior thereto, employees engaged in construction activities were exposed to injury in that the portable fire extinguisher on aerial lift #2 had a broken gauge and the pin was missing. HME Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.150(c)(1)(viii), which was contained in OSHA inspection number 1730209, citation number 1, item number 1 and was affirmed as a final order on March 28, 2024 with respect to a workplace located at Old Youngs Bridge, Astoria, Oregon 97103.

There was not at least one electrocution hazard warning conspicuously posted in the cab so that it is in view of the operator and at least two on the outside of the equipment. Barge 2250 crane - On or about May 19, 2026, and at times prior thereto, employees performing crane operations were exposed to injury in that the employer did not ensure that the required crane safety placards/decals were installed on the Manitowoc 2250 (S/N 2251008) crane. The crane was missing the power line safety warning decals required, including one decal inside the cab and one on the exterior side of the crane.

29 CFR 1926.1422: Hand signal charts must be either posted on the equipment or conspicuously posted in the vicinity of the hoisting operations. Barge 2250 crane - On or about May 19, 2026, and at times prior thereto, employees performing crane operations were exposed to injury in that the employer did not ensure that the required crane hand signal charts were properly posted as required. One side of the Manitowoc 2250 (S/N 2251008) crane displayed an outdated and unreadable hand signal chart, while the opposite side of the crane had no hand signal chart posted at all.
