29 CFR  1926.102(b)(1):  Protective eye and face protection devices must comply with any of the following consensus standards:  (i)  ANSI/ISEA Z87.1-2010, Occupational and Educational Personal Eye and Face Protection Devices, incorporated by reference in § 1926.6; (ii)  ANSI Z87.1-2003, Occupational and Educational Personal Eye and Face Protection Devices, incorporated by reference in § 1926.6; or (iii)  ANSI Z87.1-1989 (R-1998), Practice for Occupational and Educational Eye and Face Protection, incorporated by reference in § 1926.6.  (A)  Protective eye and/or face protection devices did not comply with consensus standards as required.  On or about May 28, 2026, employees were wearing sunglasses using pneumatic nail guns to install shingles on the roof of a residential home located at 1424 West Sprague Road, CLEVELAND, OH 44134. The employer did not ensure the sunglasses worn by employees complied with consensus standards as required.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 28,  2026, employees were exposed to a fall hazard of 9- 22 feet distance to the ground while completing shingle installation on a residential roof located at 1424 West Sprague Road, CLEVELAND, OH 44134, and were not protected by guardrail systems, safety net system, or personal fall arrest system.
