29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) 2315  Ave., Helena MT, 59601: On or about June 02, 2026, employees were exposed to a catapult and subsequent fall of approximately 20 feet to the ground below while installing gutter, fascia and  trim from a Genie S-65 Aerial Lift. The employees were working from the basket without and attached lanyard.
