An employee was lowering the frame of the bread rounder back into place on the conveyor and pinched their right thumb resulting in amputation of the thumb.

An employee was cutting top sirloin on a meat saw and amputated the right index finger.

An employee's gloved hand was caught between the moving rollers while removing strings from the powered conveyor resulting in the amputation of his right finger above the first knuckle.

An employee was operating an LPM machine to seal small mustard packages. A mustard package jammed up the LPN machine and when the employee tried to pull the mustard package out, part of her left index finger was removed. The machine was guarded at the time.

An employee was using a meat saw when his hand slipped and his right middle finger engaged with the blade, lacerating his fingernail and fingertip.

An employee was adjusting a press machine when the ram fell and crushed her right ring finger, amputating it at the knuckle.

An employee was getting ready to hook up to the dome while using the vault cart. The employee's left thumb was caught in the vault cart winch and the end of his thumb was amputated.

An employee suffered a right-hand middle finger amputation while cleaning a lint filter.

An employee was feeding film into a wrapping machine when her hand contacted the machine, amputating approximately three-quarters of three left-hand fingers. The machine was guarded at the time of the incident.

An employee was using a handheld skill saw when the employee's glove was caught in the blade. The employee's left ring finger was then amputated up to the first knuckle.

An employee had just finished lifting a shale bin using two loaders (one with a stinger, chain, and lifting equipment) in order to place matting underneath. Once the matting was in place, the employee directed the loader to lower the bin until there was enough slack in the lifting chain to remove it. As the employee grabbed the eye of the lifting chain to unhook it, the operator of the loader bumped the controls with his knee, causing the chain to tighten and pinch the employee's right middle finger between the bin and the chain. The employee's right middle finger was amputated between the middle and upper joint, and the employee was hospitalized.

An employee was hospitalized due to slip and fall conditions.

An employee was setting up tools on a CNC machine. He had commanded a tool change when he noticed the coolant line was in the path of the tool changer. When he attempted to move the coolant line, the tool changer went into motion. His right index fingertip was then amputated between the tool and the changer (no bone loss).

On December 27, 2016, at approximately 10:50 p.m., an employee was changing the shackles on an evisceration line when his right thumb was amputated by the line. The shackle line was not turned off at the time of the incident.

An employee was shutting down an air handler and using a screwdriver to slow the belt down when his left fourth fingertip was caught and amputated in the belt.

An employee was operating a tube winder and amputated three fingers on the left hand.

An employee was using a table saw to rip small pieces of 2x4 for a form when his left index finger contacted the running blade, amputating the first knuckle. The saw was guarded at the time of the incident.

An employee's left thumb was partially amputated when it got caught between a beam and the hook of the overhead crane used to lift the beam.
